Some adult males suffer from a rare condition known as buried penis, where the penis is covered by excess skin in the pubic area or scrotum. The penis is of normal size and function, but it is hidden beneath the skin of the abdomen, groin, or scrotum. This can cause embarrassment and psychological distress, and is usually corrected surgically.


One study in Japan found that less than 4% of newborn boys have this condition, which tends to be congenital, meaning it is present at birth and may develop later in childhood or adulthood.

Causes of a Buried Penis

There are several possible causes of a buried penis. It may be present at birth or develop later in life. This condition does not appear to be an inherited trait or related to a person's hormones. It is more commonly observed in infants and young boys than in older boys and men.


Common causes include:

1. Birth defects, resulting from abnormally weak ligaments that connect the penis to the body.


2. Obesity, as excess fat around the abdomen and genitals can cause the penis to appear buried.


3. Lymphedema, as swelling around the scrotum can cause the penis to be buried within the tissues due to the accumulation of lymphatic fluid.


4. Excessive or insufficient removal of the foreskin during circumcision.


Diagnosis of a Buried Penis

The doctor primarily relies on a physical examination for diagnosis, keeping in mind that there is a difference between a buried penis and a small penis. Therefore, the doctor must be able to clearly differentiate between the two conditions.


Treatment of a Buried Penis

Treatment varies from case to case, depending on the underlying cause. In infants and younger children, the condition sometimes resolves spontaneously. In older children and adults, weight loss may help resolve the problem. If weight loss is insufficient, surgery may be required.

Types of surgical procedures:


1. Scar tissue removal:

This involves separating the ligaments that connect the base of the penis to the pubic bone.


2. Liposuction:

This involves removing fat cells by liposuction, using a catheter to suction out the fat cells located under the skin in the area surrounding the penis.


3. Abdominoplasty:

This involves removing excess fat and skin from the abdominal area.


4. Pantyhose excision:

This involves removing excess tissue and skin that hangs over the genitals and thighs.


5. Osteotomy:

This involves removing the layer of fat over the pubic area.


6. Skin grafting:

This involves covering areas of the penis that need to be covered.


A buried penis can also be treated with medications, such as antibiotics, if an infection develops in the genital area.

Refrain from eating and drinking for at least 6 hours before the procedure.

Take a thorough shower on the day of the procedure.

Remove hair from the treatment area.

Blood and urine tests.

ECG and X-rays for those over 40.
