The most common cause of fistula formation is the development of an abscess in the perianal area. The abscess drains through an external opening through the skin, forming a gap with an internal opening that drains into the anal canal.

In most cases, opening and removing the fistula tract through a surgical procedure—a fistulotomy—is sufficient treatment.

A fistula that involves a large portion of the sphincter and musculus puborectalis can lead to complications, including fecal incontinence, if treated as described above. Therefore, it is preferable to treat such cases more conservatively.

There are several treatment options, including a seton, a surgical procedure in which a wire is inserted through the fistula tract to drain and close the fistula.

Other treatments include closing the internal fistula with a sling. Recently, fibrin glue has been used, which has been successful in closing the fistula in some cases.
